Choose a starting point
Choose one of the four routes, make your selections, then use Continue to review refinements and matching concepts.
- Assessment and measurements: explore intake, vital signs and risk scores.
- Clinical findings: explore recorded findings and observations.
- Care-related disorders: explore disorders and related conditions.
- Care procedures and interventions: explore assessment, care planning, prevention and practical care.
Combine choices and refine results
On the Disorders index, choices within one right-hand group are alternatives. A concept must match every right-hand group you select and one of the selected left-hand routes, if any. The other indexes combine alternatives within each panel and require a match in both panels when both have selections.
In Care-related disorders, Other recorded concepts selects the records outside the named routes. It clears named left selections and keeps the right-column filters. Selecting a named route clears Other recorded concepts.
Counts show matching concepts in the context of your other selections. They can change as you select choices. Zero-count choices stay visible and are inactive; a choice already selected can still be cleared.
On the refinements page, choices within a row combine with OR: a concept may match any selected choice in that row. Selected rows combine with AND: a concept must meet the choices in each selected row. Refinements always apply together with your first-page selections.
Use Reset to clear the first-page selections, or Reset refinements to clear only refinements. Continue on Refinements moves to the matches below. Use Word filters to require every entered word in a concept name.
Families and charts
Select a title and count to add that route to your selection. Where a Chart button is available in Disorders, it opens that family with the right-column filters applied. Your Page 1 selection is retained when you return. Expand branches with their arrows; select a concept for its Meaning. Meaning and Medical Context opens its full page.
Blue rows identify family headers. Counts are distinct concepts in a branch; overlapping families can share concepts. Expand all, Collapse all and Print apply to the current chart. Back from a concept restores the chart and selection. Continue shows the remaining distinct count.
Read a matching concept
Select the triangle beside a result to expand or collapse its inline Meaning. Select the concept name, or the expanded Meaning text, to open the full Meaning and Medical Context.
Concept names use the preferred term when available, otherwise the fully specified name (FSN). The full page also shows the full FSN as a subtitle to help distinguish concepts.
Move between pages
The breadcrumb trail above the navigation shows your route. Select Nursing and Clinical Care at its root to return to the starting-point choices, or select a route or refinements breadcrumb to revisit that page.
Use the Snake Back and Forward arrows in the header to revisit displayed pages and associations. The outer arrows jump to the first or last displayed page.
Select Word-family index to browse concepts by name. Choose an initial letter, enter a word or phrase, or select an indexed concept to open its page. Index search checks concept names and SNOMED descriptions; it does not search Meaning or Medical Context.
Pin a concept to keep it available while you explore other entries.
